Top nursing executive at Phoenix VA out

PHOENIX (CBS5) -

Another top member of the Phoenix VA leadership team is out. Nancy Claflin, RN, Ph.D., tendered her resignation.

An internal email obtained by CBS 5 News sent by Phoenix VA Interim CEO Glenn Costie informed the staff of Claflin's retirement. 

"Nancy had a wonderful VA career here at the Phoenix VA Health Care System for over 40 years," Costie said in the email.

Claflin's official title is Associate Director of Patient Care Services. She is the top nursing manager. No reason was given for her sudden retirement. 

The VA has confirmed that they are currently conducting a review of their nursing services to identify strengths, as well as areas that need improving.

CBS 5 News has also confirmed that the Joint commission conducted an unannounced visit to the Phoenix VA for what is called a "for cause survey." Many times, those are the result of complaints.

The VA said the results of the survey are not yet finalized.
